This page covers the INSPIRE website. It explains what we ask for, why we ask, and what happens to it after you press send.

What this site collects
When you fill out a form here, we collect what you type: your name, email address, phone number, ZIP code, age, your congregation if you name one, and your answer to the question about a dementia or other major diagnosis. If you arrive through a congregation's own link, we also record which congregation sent you, so credit for the referral lands in the right place.
We use it for one purpose: so a member of the team can contact you about taking part. It is contact and eligibility information, not a medical record, and it is not a substitute for anything your doctor keeps.
What we don't do with it
We don't sell it. We don't rent it. We don't share it with insurers, employers, or advertisers. We don't use it to target ads to you anywhere.
Who can see it
The INSPIRE team at Charles R. Drew University of Medicine and Science, and the site leads at your own congregation where you signed up through them. Access is limited to the people who need it to reach you and to run the program.
Research data is handled separately
Signing up on this site is not the same as enrolling in the research. If you go on to enroll, that happens through a separate consent process reviewed by the Charles R. Drew University Institutional Review Board, and your research records are kept under a study ID in the systems approved for that purpose — not in this website's database.
Habit tracking in the member app
The member area records whether you marked a self-care action as done. It does not store lab results, medication names, diagnoses, or clinical measurements.
How long we keep it and how to have it deleted
We keep contact records for as long as the program is actively in touch with you. Ask us to delete yours and we will, and we'll confirm when it's done. Withdrawing costs you nothing and affects nothing else.
Cookies
This site stores your language choice and your accessibility settings in your own browser so it remembers them next visit. That's local to your device.
